Welcome to Autism Allies. We offer comprehensive evidence-based Applied Behavior Analysis (ABA) services to support individuals on the autism spectrum and their families.
Please have the following documents ready to complete this form:
- Neuropsychological evaluation (preferably one done within the past three years);
- An updated physical examination;
- Copy of front side and backside of insurance card; and
- If available, any other outside provider assessments (i.e. speech, occupational therapy, physical therapy, mental health).
